The initial appointment will usually be a consultation during which time the doctor will review your medical and dental history as well as your diagnosis and treatment options. On occasion, surgery will be done the same day as the consultation. However, a complex medical history or treatment plan may require further evaluation and a second appointment may be required to provide treatment on another day.
Please assist us by providing the following information at the time of your first visit:
If a dentist has provided you with a referral slip please bring it with you.
A list of medications you may be taking.
Any medical or dental insurance forms.
If needed, a primary care referral from your physician.
If surgery is anticipated, please withhold from eating or drinking for on least six to eight hours before surgery.
If surgery is anticipated, please make arrangements to be driven home.
Any x-rays from your dentist which might be helpful.
IMPORTANT: ALL PATIENTS UNDER THE AGE OF 18 YEARS MUST BE ACCOMPANIED BY A PARENT OR LEGAL GUARDIAN.
Please alert the office if you have any medical condition prior to surgery. We especially need to know if you have a heart condition, diabetes, high blood pressure, seizure disorder, thyroid condition, liver disease, bleeding disorder, artificial heart valves and joints, rheumatic fever or you are taking any medications, including aspirin, immunosupressive drugs, prednisone, anticoagulant therapy or antiviral drugs.
If your dentist or physician has taken x-rays you may request they be sent to our office. If there is not enough time for them to be sent to us via mail, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our office. If your dentist or physician has access to a scanner and the Internet, x-rays can now be e-mailed directly to us through our secure Web site.
