One of the most persistent problems patients face is determining what is and isn't covered under their insurance policies. There is tremendous variability in the policies written not only by different insurance companies but also with policies written for different employers by the same company.

Every plan written for a specific employer has different levels of coverage and eligibility requirements. Adding to this confusion is that some procedures we perform may actually be covered only by your medical insurance and some only by your dental insurance.

TO BETTER ASSIST YOU IN UNDERSTANDING YOUR INSURANCE PLEASE BRING YOUR MEDICAL AND DENTAL INFORMATION ON YOUR FIRST VISIT TO US.

After your initial consultation, our office will contact your insurance carrier(s) to determine the level of coverage available under your specific plan(s). With that information, we will estimate what portion of our fees may become your financial responsibility. We will then contact you with this information before any additional treatment takes place.

Our office policy is to collect your estimated financial responsibility for treatment on the day that the treatment is scheduled to take place. It is important to note that this estimated figure is based on information provided to our office by your insurance carrier(s). It is not guaranteed. If it is later determined that your responsibility should have been more than what was quoted, you would receive a bill from our office. Likewise, if it is determined that your responsibility should have been less, you will receive a refund for your overpayment.

Please remember, ultimately you, not your insurance company, are fully responsible for all fees charged by this office. Payment is expected at the time of service unless other arrangements have been made in advance.
